With multiple therapies now available, the discussion centers on treatment prioritization based on somatostatin receptor status, tumor grade, and burden. High-grade, symptomatic tumors may benefit from early chemotherapy, while grade 1 or 2 somatostatin receptor-positive tumors are candidates for early peptide receptor radionuclide therapy (PRRT), particularly after somatostatin analog therapy. The NETTER-2 trial (NCT03972488) updates on the efficacy of PRRT in grade 2 NET reinforce these treatment guidelines.
