ASCO 2025 | The future of HER2-directed therapies in non-small cell lung cancer

Joshua Sabari, MD, New York University Langone Health, New York, NY, provides an overview of the growing options in the HER2 space for non-small cell lung cancer (NSCLC). Zongertinib stands out due to its clean side effect profile, with minimal toxicity, no risk of interstitial lung disease, and no significant gastrointestinal or dermatological side effects. The approval of trastuzumab deruxtecan will also provide patients with much needed options. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.

We just recently saw data from BAY 292 and we have an approval for T-DXd, an accelerated approval in the second line setting and HER2 being an ADC targeting HER2. So with multiple options, I think it’s critical to talk to patients about what the side effect profiles are. zongertinib really differentiates itself in that it is clean, it has very little toxicity for patients, no risk of interstitial lung disease, we don’t really see many significant AST and ALT elevation or abnormalities, and most importantly to me, no real diarrhea and no real significant rash. So these patient reported outcomes are going to be critical in differentiating the different therapeutic options for patients.
