ASCO GI 2026 | ctDNA as prognostic marker in CRC: high predictive value, assay limitations

Emerik Osterlund, MD, PhD, The University of Texas MD Anderson Cancer Center, Houston, TX, discusses the use of circulating tumor DNA (ctDNA) in colorectal cancer (CRC). ctDNA is an established prognostic marker. While long-recognized as highly prognostic for disease monitoring, limitations remain. More sensitive assays are needed to reliably confirm disease absence. However, positive predictive value is excellent: when ctDNA is detected, recurrence is highly likely. Challenges persist in developing better assays to confidently confirm disease-free status. This interview took place at the 2026 American Society of Clinical Oncology Gastrointestinal Cancers Symposium in San Francisco, CA.
