Mark Opdam, The Netherlands Cancer Institute, Amsterdam, Netherlands, discusses the potential role of immunohistochemical (IHC) biomarkers in supporting chemotherapy de-escalation strategies for early breast cancer. Drawing from a biomarker analysis to the MATADOR trial (ISRCTN61893718), he highlights the established association between high stromal tumor-infiltrating lymphocytes (sTILs) and favorable outcomes in triple-negative breast cancer, alongside emerging signals that high EZH2 expression may also correlate with improved response to taxane-based regimens. While not yet ready for clinical application, these findings suggest that IHC-based tools could contribute to more personalized treatment strategies in the future.
