ESMO 2024 | DESTINY-Breast04: T-DXd in HER2-low breast cancer regardless of molecular markers

Naoto Ueno, MD, PhD, University of Hawai’i Cancer Center, Honolulu, HI, discusses the results of the DESTINY-Breast04 study (NCT03734029), which compared trastuzumab deruxtecan (T-DXd) to physician’s choice of chemotherapy in patients with HER2-low metastatic breast cancer. The study highlights the consistent superiority of T-DXd over standard treatments, irrespective of molecular markers like DNA repair dysregulation or HER2 expression. While higher HER2 expression and total lymphocyte infiltration in the tumor microenvironment show potential trends of benefit, these findings are hypothesis-generating, and T-DXd remains effective across diverse molecular backgrounds.
