ASCO 2025 | OS and biomarker results of the NURE-Combo trial in MIBC

Chiara Mercinelli, MD, IRCCS San Raffaele Hospital, Milan, Italy, discusses the Phase II NURE-Combo trial (NCT04876313) of neoadjuvant nivolumab plus nab-paclitaxel followed by surgery and adjuvant nivolumab in patients with muscle-invasive bladder cancer (MIBC). Long-term follow-up showed sustained survival benefits and acceptable tolerability, and molecular analysis indicated better responses in non-luminal subtypes. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.

NURE-Combo trial is a phase 2 trial conducted entirely in our hospital in Milan. We tested 4 cycles of neoadjuvant immunotherapy plus Nab-Paclitaxel, followed by radical cystectomy and then 1 year of adjuvant nivolumab. The first results of NURE-Combo trial have already been published on JCO last year, showing positive results in terms of pathological complete response rate. Now here at ASCO we are presenting updated results after 24 months of follow-up with positive overall survival data, in particular after 24 months, 89% of patients were still alive. And we also presented some biomarker analysis conducted in collaboration with Veracyte. So we did a transcriptomic analysis on tumor tissue samples of patients obtained at baseline showing that non-luminal subtypes tend to have better pathological complete response rates.
