ASCO 2025 | Predictive biomarkers for T-DXd response in metastatic breast cancer

Paolo Tarantino, MD, Dana-Farber Cancer Institute, Boston, MA, comments on a quantitative pre-treatment evaluation of HER2 and topoisomerase 1 (Topo1) expression as predictors of response to trastuzumab deruxtecan (T-DXd) in patients with metastatic breast cancer. Using HER2DX and RPPA assays, higher HER2 mRNA and protein levels were associated with longer time to next treatment in both HER2-positive and HER2-negative disease. Conversely, elevated Topo1 protein levels correlated with poorer outcomes in HER2-negative cases, supporting biomarker-driven treatment strategies. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.
