ASCO 2025 | Evaluating the emerging role elraglusib as a treatment option in mPDCAC

Devalingam Mahalingam, MD, PhD, Northwestern University, Evanston, IL, briefly discusses the potential of elraglusib in advancing the treatment of first-line metastatic pancreatic ductal adenocarcinoma (mPDAC). While this exciting new agent may increase the efficacy of commonly used chemotherapy strategies, a greater understanding of the underlying mechanisms and Phase III trials are needed to confirm its potential. This interview took place at the 2025 American Society of Clinical Oncology (ASCO) meeting in Chicago, IL.

I think we’ve been working with two chemotherapy backbones for now a decade. I think in most patients this is the standard treatment that most physicians when we are seeing a patient we are trying to decide should we use FOLFIRINOX or should we use GEM, NAB, Paclitaxel. I think both drugs are equally comparable. We know that they are molecularly targeted therapies in a subset of patients and for those patients we certainly have options, either on completion of first line with the PARP inhibitors, especially if they are germline BRCA1, BRCA2, or if they have other alterations that are found can be used on progression on the frontline setting...

But we really haven’t found an agent that can kind of immunomodulate and allow chemotherapy to work better. And I think, you know, and this is a new class of agent. I think the excitement is about the class of agent. We need to tease out a little bit more about how the true mechanism of the drug and then also start planning the kind of like confirmatory or what we call registration trials, registration phase three trials to try and get elraglusib out to patients sooner rather than later if it proves to be positive in a subsequent trial.
