Ahmad Tarhini, MD, PhD, H. Lee Moffitt Cancer Center, Tampa, FL, discusses the Phase II ECOG-ACRIN EA6194 trial (NCT04708418), which evaluated neoadjuvant pembrolizumab alone or with intratumoral vidutolimod in patients with resectable stage IIIB–D cutaneous melanoma. The combination demonstrated an acceptable safety profile and a higher rate of major pathologic responses compared with pembrolizumab alone. These findings suggest that adding vidutolimod may enhance neoadjuvant efficacy without substantially increasing toxicity, supporting continued investigation in this patient population.
