ESMO 2025 | Dose-escalation and expansion study of 225Ac-J591 in mCRPC

Scott Tagawa, MD, MS, FACP, Weill Cornell Medical Center, New York, NY, comments on results from a Phase I dose-escalation and expansion trial (NCT04506567) evaluating fractionated and multiple-dose regimens of the PSMA-targeted alpha radioligand 225Ac-J591 in patients with metastatic castration-resistant prostate cancer (mCRPC). The fractionated regimen demonstrated manageable safety and preliminary efficacy, with thrombocytopenia identified as the main dose-limiting toxicity. Multiple-dose schedules were less feasible due to hematologic timing, and a multicenter validation study is ongoing. This interview took place at the European Society for Medical Oncology (ESMO) 2025 Congress in Berlin, Germany.
