David Sebag-Montefiore, MBBS, MRCP, University of Leeds, Leeds, UK, discusses the challenges and opportunities of management and treatment of older patients with rectal cancer, which represents a large and growing proportion of rectal cancer patients. Treatments have typically been tested in younger, fitter patients, meaning there is limited evidence for therapies in older patients, who are frailer and can lack the capacity to withstand toxicities. Management in older patients therefore requires adaption to the patient, taking into consideration their fitness and preferences. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2026 Congress in Munich, Germany.
