Umair Mahmood, BSc, MSc, University College London Hospital, London, UK, discusses the potential of POLE mutations as predictive biomarkers for immune checkpoint inhibitor therapy in gastrointestinal cancers. In a study, patients with colorectal or gallbladder cancer harboring POLE mutations experienced durable responses to immunotherapy despite differing microsatellite stability and tumor mutational burden profiles. These hypothesis-generating findings suggest POLE alterations may help identify patients likely to benefit from immune checkpoint inhibitors and support prospective studies to validate their clinical utility.
