WCLC 2026 | STARLET: osimertinib alone or with SRS for EGFRm lung cancer with brain metastasis

Cheryl Ho, MD, FRCPC, BC Cancer, Vancouver, Canada, Yu Yang Soon, BSc, MBBS, MSc, FRANZCR, National University Cancer Institute Singapore, Singapore, Singapore, Fiona Hegi-Johnson, MBBS(Hon), BSc, Med Hons, FRANZCR, PhD, Peter MacCallum Cancer Center, Victoria, Australia, shares insights into the STARLET pooled analysis of the LUOSICNS (NCT03769103) and OUTRUN (NCT03497767) trials to investigate whether the addition of stereotactic radiosurgery (SRS) to osimertinib improves outcomes for patients with EGFR-mutated (EGFRm) lung cancer with brain metastasis. The primary outcome of 12-month progression-free survival did not show a statistically significant difference between osimertinib alone versus osimertinib with SRS and secondary endpoints of tumor kinetics were also similar between groups. Quality-of-life outcomes were similarly consistent across the two groups, supporting the use of osimertinib alone followed by SRS at intercranial progression for EGFR-mutated lung cancer in patients with minimally symptomatic brain metastasis. This interview took place at the IASLC 2026 World Conference on Lung Cancer congress in Seoul, Republic of Korea.
