ESMO 2026 | Regulatory T cells and the immune compartment in pre-invasive lung lesions

James Reading, PhD, University College London, London, UK, discusses the identification of pre-invasive lesions in high-risk lung cancer patients and the characterization of the immune compartment within these lesions, revealing a subset of regulatory T cells that are potent immunosuppressive cells. Dr Reading highlights that targeting these regulatory T cells with a PI3Kδ inhibitor can dismantle the pre-invasive immune regulatory axis, leading to a significant reduction in lung tumor development in a mouse model, and that a prototype biomarker has been identified in the blood to track the formation of lung cancer and select patients who may benefit from Treg-targeted immunotherapy. This interview took place at the European Society for Medical Oncology (ESMO) 2026 Congress in Madrid, Spain.
