SABCS 2025 | Guideline-based sequencing in metastatic HER2-positive breast cancer

Azka Ali, MD, Cleveland Clinic, Cleveland, OH, comments on current guideline-based recommendations for metastatic HER2-positive breast cancer, highlighting the standard regimen of taxane chemotherapy with trastuzumab and pertuzumab. Additionally, for patients with recurrent breast cancer within 12 months, a second-line antibody-drug conjugate like trastuzumab-deruxtecan is often used. The DESTINY-Breast09 trial (NCT04784715) has shown improved outcomes with trastuzumab deruxtecan plus pertuzumab, making it a suitable option for select high-risk patients, although it is not currently approved for first-line use.
