ASCO 2025 | Updates in immunotherapy and BRAF inhibition for melanoma at ASCO 2025

Sarah Danson, PhD, FRCP, MSc, MBBS, University of Sheffield, Sheffield, UK, provides an overview of recent updates in melanoma. She comments on the Phase III DREAMseq trial (NCT02224781), which assessed first-line treatment with nivolumab plus ipilimumab in patients with previously untreated BRAFV600-mutant metastatic melanoma, as well as the role of low dose immunotherapies. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.

In the same session that I was in, the DREAMseq mature data was presented, which definitively shows that in BRAF mutation positive patients, that giving the immunotherapy first off is the best thing to do, which is good because actually in Dante, we found that that was done in the vast majority of cases. But I think, you know, we do need to keep pushing that message that BRAF inhibition is really a second line treatment unless the patient’s very sick to begin with...

In the same session that I was in, the DREAMseq mature data was presented, which definitively shows that in BRAF mutation positive patients, that giving the immunotherapy first off is the best thing to do, which is good because actually in Dante, we found that that was done in the vast majority of cases. But I think, you know, we do need to keep pushing that message that BRAF inhibition is really a second line treatment unless the patient’s very sick to begin with. And then, yes, the other thing that was of interest to me was the low dose immunotherapy treatment, which, you know, if we could be giving low doses and reducing the financial toxicity, making it more available to patients, then I think that would change a huge number of things as well. So I need to go away and really scrutinize that data too.
