ASCO 2023 | Opting for watch and wait: non-operative strategy for locally advanced rectal cancer with CR

Jose G. Guillem, MD, MPH, MBA, FACS, FASCRS, University of North Carolina, Chapel Hill, NC, discusses the concept of a “watch and wait” approach for patients with locally advanced rectal cancer who have undergone preoperative chemoradiation therapy. Data now provides reassurance that patients with a complete clinical response, as determined by various parameters, can opt for a watch and wait strategy instead of immediate surgery. This decision should be made with a well-informed multidisciplinary team considering the potential benefits, challenges, and consequences. While the approach may not be suitable for every patient, it offers a viable option for select cases. This interview took place at the American Society of Clinical Oncology (ASCO) 2023 Annual Congress in Chicago, IL.
