ASCO 2023 | Watch and wait vs. surgery: shared decision-making in locally advanced rectal cancer treatment

Jose G. Guillem, MD, MPH, MBA, FACS, FASCRS, University of North Carolina, Chapel Hill, NC, addresses the challenging question of whether to pursue a “watch and wait” approach or opt for surgery in patients with specific goals and expectations regarding their treatment. The decision involves shared decision-making among the patient, their family, and multiple healthcare providers. The multidisciplinary team plays a crucial role in informing patients about the risks and benefits of both options. Minimally invasive surgery, such as robotics, can help minimize the risks of operative approaches. Prof. Guillem emphasized that ultimately, the decision should be carefully considered on an individual basis, taking into account the patient’s unique circumstances and preferences.
