ASCO 2026 | Evaluating cabozantinib monotherapy in high-grade neuroendocrine neoplasms

This study looked into the very high-grade population, patients who have aggressive tumors and usually have a survival of less than one year. It recruited patients who have received prior treatments and essentially allocated them to cabozantinib once daily. It was non-randomized, it was non-blinded. About 32 patients were accrued and the results were presented yesterday. Essentially five out of them, almost 16%, have a partial response rate and that is in a population where about one third had extremely high grade disease, poorly differentiated disease. Of our five responses, three had a small cell of poorly differentiated histology, something that has never been described for a small molecule agent, for an oral agent. So this is a trial that showed some efficacy in a subset of patients with high grade disease, a PFS of about nine months and an overall survival of nine months in heavily pretreated patients. So we’re very excited about those results and definitely feel that cabozantinib at some point will have a part in the treatment of patients with high-grade disease.
