ASCO GI 2025 | Evaluating pCR as a surrogate endpoint for OS in esophageal cancer

Jun Okui, MD, MPH, Keio University School of Medicine, Tokyo, Japan, presents findings from an international collaborative study investigating whether pathological complete response (pCR) can serve as a surrogate endpoint for overall survival (OS) in esophageal cancer. The study analyzed a large patient population across several countries who underwent neoadjuvant chemotherapy or chemoradiotherapy followed by surgery. Results showed a weak correlation between pCR and OS, suggesting that while pCR can stratify prognosis, it may not reliably substitute for OS as a primary endpoint in clinical trials. These findings provide valuable insights for optimizing future trial designs and treatment benefit predictions. This interview took place at the ASCO GI Cancers Symposium 2025 in San Francisco, CA.
