ESMO 2025 | EMERALD: eribulin plus dual HER2 blockade in first-line HER2+ breast cancer

Shigehira Saji, MD, PhD, Fukushima Medical University, Fukushima, Japan, comments on discusses final survival results from the EMERALD study (NCT03264547) evaluating eribulin or taxane in combination with trastuzumab and pertuzumab as first-line therapy for locally advanced or metastatic HER2-positive breast cancer. Overall survival was similar between treatment arms, with median survival exceeding six years. Exploratory circulating tumor DNA (ctDNA) analyses showed PIK3CA mutation as a poor prognostic factor, though it did not influence relative treatment benefit. This interview took place at the European Society for Medical Oncology (ESMO) 2025 Congress in Berlin, Germany.

EMERALD study is our phase three study, which was done in Japan. This is for HER2 positive metastatic breast cancer patients as a first-line setting. And we evaluate the comparison between Herceptin Trastuzumab-Pertuzumab-Taxane versus Trastuzumab-Pertuzumab-Eribulin. And this is a non-inferiority trial which is showing that Eribulin arm can work very well, almost the same as Taxane arm...

And this is a non-inferiority trial which is showing that Eribulin arm can work very well, almost the same as Taxane arm. So, today and tomorrow, we will present about the OS result. And OS result is almost, actually, as we expected, expected hazard ratio is 1.25 and this is almost equivalent and we checked the biomarker say which is HER2 amplification and the PIK3CA mutation and there are much more HER2 mutations in Taxane arms so it might contribute to the little bit better OS in Taxane arms, but in general, actually we can show the Eribulin, Trastuzumab-Pertuzumab-Eribulin works very well in terms of OS also.
