ESMO 2023 | PSA rise factors & significance of doubling time

Bertrand Tombal, MD, PhD, from the Université catholique de Louvain in Ottignies-Louvain-la-Neuve, Belgium, discusses the factors influencing PSA rise and the significance of PSA doubling time post-prostatectomy and radiation therapy. For patients experiencing a rise in PSA, distinguishing those at risk of developing metastasis is crucial. Gleason score, timing of recurrence, and PSA doubling time are key factors. Patients with a high Gleason score (eight or above) and a shorter time to recurrence face higher risks. The dynamic component lies in the speed of PSA rise, measured by doubling time. A slow rise suggests a low risk of metastasis, while a rapid increase (doubling time less than one year) demands attention and potential intervention. Prof. Tombal highlights the importance of PSA doubling time in guiding appropriate actions, emphasizing the risk of overtreatment in certain cases.
