Guglielmo Vetere, MD, The University of Texas MD Anderson Cancer Center, Houston, TX, discusses immunotherapeutic stategies to overcome the immunologically “”cold”” tumor microenvironment in microsatellite stable colorectal cancer (CRC). Interest focuses on non-liver metastasis patients, testing next-generation PD-1/CTLA-4 agents and targeted therapy combinations. The STELLAR-303 trial (NCT05425940) showed multi-tyrosine kinase inhibitor plus immunotherapy was superior to regorafenib. However, specific subpopulations achieving clinically meaningful benefit remain unclear, and require further investigation.
