Alexandra Drakaki, MD, PhD, University of California Los Angeles, Los Angeles, CA, discusses an exploratory analysis of plasma circulating tumor DNA (ctDNA) from the Phase III VOLGA (NCT04960709) safety run-in (SRI) cohort. This study assessed ctDNA dynamics in cisplatin-ineligible patients with muscle-invasive bladder cancer (MIBC) undergoing neoadjuvant durvalumab, tremelimumab, and enfortumab vedotin therapy before radical cystectomy. ctDNA clearance during treatment was associated with improved pathologic response and event-free survival (EFS). These promising results suggest ctDNA clearance as a potential surrogate for treatment efficacy, warranting further investigation in the ongoing VOLGA study.
