Hyung-Don Kim, MD, PhD, Asan Medical Center, Seoul, South Korea, discusses the challenges of first-line immunotherapy for metastatic gastric cancer. While it is the standard of care, many patients either do not respond initially or develop resistance over time, and no biomarkers have been identified to represent resistance signals in gastric cancer. Dr Kim presents a study exploring the role of translationally controlled tumor protein (TCTP), recently recognised for its role in driving immunosuppressive gradient in the tumor microenvironment. Higher TCTP levels correlate with poorer outcomes in patients with gastric cancer receiving either chemotherapy or immunotherapy, highlighting TCTP as a promising biomarker for monitoring and overcoming immunotherapy resistance.
