ASCO 2025 | STAR-EV: neoadjuvant enfortumab vedotin and SBRT in MIBC

Tian Zhang, MD, UT Southwestern Medical Center, Dallas, TX, discusses the Phase I/II STAR-EV trial (NCT06394570) of neoadjuvant enfortumab vedotin with stereotactic body radiotherapy (SBRT) in patients with localized, cisplatin-ineligible muscle invasive bladder cancer (MIBC). This study assesses safety, feasibility, and pathologic complete response rate prior to cystectomy. Patients receive up to three cycles of treatment with escalating timing of radiotherapy. The study also investigates pathologic downstaging, treatment-related toxicities, surgical timing, and exploratory biomarkers. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.
