WCLC 2025 | Identifying high-risk patients with NSCLC excluded from screening programs

Laura Mezquita, MD, PhD, Hospital Clinic – IDIBAPS, Barcelona, Spain, comments on findings from a prospective study of 350 patients with non-small cell lung cancer (NSCLC) evaluating characteristics of those not meeting standard lung cancer screening (LCS) criteria. Over 60% were LCS-ineligible due to age, smoking history, or never-smoking status. This group showed higher prevalence of driver mutations, advanced-stage diagnoses, and strong familial cancer history, particularly among non-smokers. The results highlight the need to refine LCS strategies to better capture high-risk populations beyond tobacco exposure. This interview took place at 2025 World Conference on Lung Cancer (WCLC) in Barcelona, Spain.

So this is a very hot topic right now because there is a substantial proportion of patients that we are not able to catch in the current lung cancer screening programs. So using the data from patients enrolled in the SCAN project, so it’s consecutive patients with non-small cell lung cancer that we collect prospectively the family history, the family history of cancer, personal history of cancer, exposure to other risk factors. So we try to identify the criteria specifically in this population. So if female patients or patients with a family history of cancer or personal history of cancer and this kind of risk factors can provide additional information to complement smoking. Now that it’s very limited to catch these patients to improve and redefine the lung cancer criteria for screening.
