ESMO GI 2026 | SIERRA: STRIDE as a first-line therapy in poor-prognosis patients with advanced uHCC

Lorenza Rimassa, MD, Hunimed University, Milan, Italy, shares insight into the Phase IIIb SIERRA study (NCT05883644), which is evaluating single tremelimumab regular interval durvalumab (STRIDE) as a first-line therapy in patients with advanced unresectable hepatocellular carcinoma (uHCC). This study included patients that had been excluded from the registration study for STRIDE, including those with less well-preserved liver function, poorer clinical conditions and with portal vein thrombosis. The safety profile was found to be consistent with previous data, but slightly worse efficacy was observed, which may be caused by the decreased prognosis of the patients rather than the efficacy of the combination. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2026 Congress in Munich, Germany.
