Umair Mahmood, BSc, MSc, University College London Hospital, London, UK, comments on the biological and clinical implications of POLE mutations as potential predictive biomarkers for immune checkpoint inhibitor therapy in gastrointestinal cancers. Both exonuclease domain and non-exonuclease domain POLE variants were associated with durable responses in this case series, supporting their potential role in patient selection. Liquid biopsy, multidisciplinary decision-making, and individualized treatment strategies are needed to optimize immunotherapy duration and surgical management in this rare molecular subgroup.
