Terry Mamounas, MD, MPH, FACS, from the AdventHealth Cancer Institute, Orlando, FL, discusses the duration of endocrine therapy in the context of locoregional treatment de-escalation. Extended endocrine therapy beyond five years can lead to significant improvements in outcome, but the absolute differences in outcome are modest, and not all patients benefit from extended therapy. Tailoring extended endocrine therapy to individual patient characteristics, such as age, tumor size, and nodal status are important, and genomic classifiers and circulating tumor cells may help identify patients who would benefit from extended therapy. This interview took place at the 2025 St. Gallen International Breast Cancer Consensus Meeting in Vienna, Austria.
