Muscle-invasive bladder cancer (MIBC) with variant histologies poses treatment challenges due to poor responses to standard therapies and limited trial data. In this latest episode of the VJOncoAlert Journal Club, Petros Grivas, MD, PhD, University of Washington School of Medicine, Seattle, WA, discusses findings of a Phase II study of neoadjuvant pembrolizumab plus accelerated MVAC chemotherapy in patients with MIBC. This combination achieved a 53% pathologic complete response rate in patients with nonurothelial subtypes, with encouraging survival outcomes. Despite toxicity concerns, findings suggest chemoimmunotherapy may be a promising strategy for this high-risk population, warranting further investigation.
