ASCO 2026 | BRAF/MEK inhibition prior to immunotherapy in melanoma: insights from the COWBOY trial

Kalijn Bol, MD, Radboud University Medical Center, Nijmegen, Netherlands, presents findings from the Phase II COWBOY trial (NCT02968303) examining induction vemurafenib with cobimetinib prior to immunotherapy with nivolumab and ipilimumab in patients with BRAF V600E/K mutated melanoma and elevated LDH compared with upfront immunotherapy. The trial was terminated prematurely due to slow patient accrual, however analysis from the more than 70 randomized patients showed a lower response rate in the induction group compared with the upfront immunotherapy group. Survival curves for progression-free survival and overall survival also favor upfront immunotherapy. This interview took place during the 2026 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.
