ASCO 2026 | COWBOY trial: optimal sequencing of BRAF/MEK inhibition and immunotherapy in BRAF-mutated melanoma

Kalijn Bol, MD, Radboud University Medical Center, Nijmegen, Netherlands, explores the optimal sequencing of BRAF/MEK inhibition and immunotherapy in patients with BRAF mutated melanoma. Unfavorable results from the COWBOY (NCT02968303), SECOMBIT (NCT02631447) and EBIN (NCT03235245) trials of induction BRAF/MEK inhibition prior to immunotherapy suggests that this treatment sequence will not benefit patients, however Dr Bol highlights that in patients with higher tumor load based on LDH, BRAF/MEK inhibition may have been required for a longer period than the COWBOY trial examined to provide benefit. This interview took place during the 2026 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.
