Iris Zalaudek, MD, PhD, University of Trieste, Trieste, Italy, discusses the challenge of overdiagnosis in early-stage melanoma and the need for a more refined follow-up strategy. Current guidelines recommend biannual follow-ups for patients with stage 1A melanoma in the first five years, followed by annual visits. However, with rising patient numbers and limited dermatology resources, risk-based assessments are essential to identify those needing lifelong surveillance. Prof. Zalaudek highlights a study from Florence identifying clinical features of thin melanomas prone to metastasis, which may guide tailored follow-up strategies.
