Félix Guerrero-Ramos, MD, PhD, Hospital Universitario 12 de Octubre, Madrid, Spain, discusses the continued importance of bacillus Calmette-Guérin (BCG) in treating high-risk non–muscle-invasive bladder cancer (NMIBC), including carcinoma in situ (CIS). BCG can remain effective in reducing disease progression despite evolving guideline classifications, and trials evaluating BCG in combination with novel agents such as subcutaneous sasanlimab point to a future where BCG continues to serve as a foundational therapy alongside new treatment strategies.
