ASCO 2025 | Improvements in outpatient follow-up of lifileucel in melanoma treatment

James Smithy, MD, MHS, Memorial Sloan Kettering Cancer Center, New York, NY, notes that the use of lifileucel, a tumor infiltrating lymphocyte (TIL), for melanoma has led to a shift in clinical practice. Some centers are adopting a less intensive outpatient follow-up, as the risk of late toxicity in TILs is rare, which is consistent with clinical experience. This approach may make TILs more accessible and improve patients’ quality of life after hospital discharge. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.

As lifileucel has rolled out across the United States after the FDA approval in 2022, a lot of cellular therapy centers have used some of their practices for CAR T patients in monitoring these TIL patients as well. For us, that meant multiple checkpoints per week in an outpatient day hospital. Because with CAR T, there’s a real concern for late toxicity with new CRS, new neurotoxicity. What these data show and what’s consistent with our clinical experience is that those events are very rare in the TIL population. So like a lighter touch or less intensive outpatient follow-up may be appropriate for these patients as well. And I think this is really important for both physicians and patients because it makes TIL maybe more accessible and improves patients’ quality of life after hospital discharge.
