ESMO 2025 | KC-WISE: anbenitamab (KN026) in pre-treated HER2+ G/GEJC

Kohei Shitara, MD, National Cancer Center Hospital East, Kashiwa, Japan, discusses the KC-WISE trial (NCT05427383) of KN026, a bispecific antibody targeting two HER2 epitopes, for HER2-positive gastric or gastroesophageal junction cancer (G/GEJC) in the second-line setting. The agent demonstrated longer overall survival and progression-free survival with an acceptable safety profile. This interview took place at the European Society for Medical Oncology (ESMO) 2025 Congress in Berlin, Germany.

Another important study in gastric cancer is a Chinese study to test KN026 as a bi-specific agent to bind two HER2 epitopes. This is a second-line trial for HER2-positive population to compare bi-specific agent KN026 in combination with second-line chemo versus chemotherapy alone. It showed remarkably longer overall survival and PFS, statistically significant and very large difference. And safety profile seemed to be acceptable with around 20% increase of diarrhea, but mostly low grade. Although this is only a Chinese-specific trial, but it is very important findings and it may support this kind of bispecific agent for HER2 epitope. So based on this rationale, we are hopefully waiting for the first-line zaniratamab trial, which combines chemotherapy with a checkpoint inhibitor. And zaniratamab is a very similar compound to KN026. And we also have another HER2 agent like HL022, which targets a different HER2 epitope. So the HER2 first-line space is somehow crowded, but this KN026 study suggests we still have room to optimize how to target it and its development in gastric cancer.
