ESMO 2024 | Treatment de-escalation and escalation strategies in HER2-positive breast cancer
Maria Vittoria Dieci, MD, PhD, University of Padova, Padova, Italy, discusses the evolving strategies in managing HER2-positive breast cancer, particularly focusing on treatment de-escalation for patients with Stage I disease. Dr. Dieci highlights how patients with small, node-negative tumors can avoid anthracyclines as part of chemotherapy. Dr. Dieci also addresses treatment escalation for high-risk patients, emphasizing the advances in clinical practice, such as the use of trastuzumab emtansine (T-DM1) as adjuvant therapy in those who do not achieve a pathological complete response (pCR) after neoadjuvant chemotherapy and anti-HER2 therapy. Looking ahead, she notes ongoing trials investigating more potent antibody-drug conjugates (ADCs) like trastuzumab deruxtecan, in both neoadjuvant and adjuvant settings.
