GU Cancers 2025 | Hurdles in using TKIs to treat patients with adrenal cortical carcinoma
Matthew Campbell, MD, MS, The University of Texas MD Anderson Cancer Center, Houston, TX, discusses the role of tyrosine kinase inhibitors (TKIs) in adrenal cortical carcinoma (ACC). Mitotane is a commonly used drug which induces CYP384, correlating increased metabolism of most TKIs. A study with sunitinib in patients with ACC found little adrenal cancer activity, low pharmacokinetic (PK) levels associated with patients exposed to mitotane. A Phase II study (NCT03370718) with cabozantinib resulted in a modest progression-free survival (PFS), demonstrating the importance of mitotane exposure in next-generation trials.
