ESMO 2026 | OPTIMUM: maintenance olaparib ± durvalumab in DDR-mutated advanced BTC

Changhoon Yoo, MD, PhD, Asan Medical Center, Seoul, South Korea, discusses results from the Phase II OPTIMUM study (NCT05222971), which evaluated maintenance olaparib with or without durvalumab in patients with advanced biliary tract cancer (BTC) harboring DDR mutations. Progression-free survival rates were similar between arms, and patients with a core homologous recombination deficiency profile derived notably better outcomes. No new safety signals emerged, supporting further development of homologous recombination deficiency-targeted therapy in this setting. This interview took place at the European Society for Medical Oncology (ESMO) 2026 Congress in Madrid, Spain.
