GU Cancers 2025 | COSMIC-313: final results of first-line triplet therapy in advanced RCC

Tian Zhang, MD, UT Southwestern Medical Center, Dallas, TX, discusses the final results from the Phase III COSMIC-313 trial (NCT03937219) of cabozantinib with nivolumab and ipilimumab as first-line treatment for advanced renal cell carcinoma (aRCC). While the combination maintained a progression-free survival and response rate advantage over nivolumab and ipilimumab alone, overall survival was comparable between groups. Safety remained consistent with previous findings. Exploratory biomarker analysis suggested improved survival in patients with high M2 macrophage abundance, highlighting potential predictive factors for treatment benefit. This interview took place at the ASCO GU Cancers Symposium 2025 in San Francisco, CA.
