Sunil Badve, MD, FRCPath, Emory School of Medicine, Atlanta, GA, discusses a secondary analysis of the ECOG-ACRIN EA1181/CompassHER2 pCR trial (NCT04266249) evaluating stromal tumor-infiltrating lymphocytes (TILs) as predictors of pathologic complete response in stage II/III HER2-positive breast cancer treated with taxane, trastuzumab, and pertuzumab (THP). Higher baseline TIL levels were consistently associated with increased pCR rates, including in hormone receptor-positive disease, supporting a key role for immune contexture and the potential use of TILs to inform neoadjuvant treatment optimization.
