Terry Mamounas, MD, MPH, FACS, from the AdventHealth Cancer Institute, Orlando, FL, comments on the trend of de-escalating surgical therapy in breast cancer, particularly in patients with low-risk tumors, where sentinel lymph node biopsy may not be necessary. Prospective randomized clinical trials have shown that omitting sentinel lymph node biopsy does not affect local recurrence in the axilla, and that this approach requires careful balancing with systemic therapy to avoid omitting potentially beneficial treatments. This interview took place at the 2025 St. Gallen International Breast Cancer Consensus Meeting in Vienna, Austria.
