Declan Murphy, FRACS, FRCS, Peter MacCallum Cancer Centre, Melbourne, Australia, explains the rationale for skipping extended pelvic lymph node dissection in high-risk patients with prostate cancer with negative prostate-specific membrane antigen (PSMA) positron emission tomography/computed tomography (PET/CT) scans. Pelvic lymph node dissection does not improve survival and increases complications. The 2024 European Association of Urology guidelines now reduce recommendations for lymph node dissection, particularly in intermediate-risk cases. PSMA PET/CT is advocated to guide decisions and avoid unnecessary procedures. This interview took place at the 2024 Advanced Prostate Cancer Consensus Conference (APCCC) in Lugano, Switzerland.
