Myriam Chalabi, MD, Netherlands Cancer Institute, Amsterdam, Netherlands, discusses the results of the NICHE-2 (NCT03026140) and NICHE-3 (NCT03026140) trials, which evaluate immunotherapy in MMR-deficient (dMMR) colon cancer. In NICHE-2, the findings suggest circulating tumor DNA (ctDNA) may guide organ preservation strategies in the future. NICHE-3, using a different immunotherapy combination (nivolumab and relatlimab), demonstrated similar results. Safety profiles were comparable, though NICHE-3 had a slightly higher rate of mild endocrine-related side effects. Both trials underscore the strong response of MMR-deficient colon cancer patients to short-term immunotherapy before surgery.
