Tom Newsom-Davis, BSc, MBBS, PhD, FRCP, Chelsea and Westminster Hospital NHS Foundation Trust, London, UK, comments on the challenges and future directions of antibody drug conjugates (ADCs) in cancer therapy, highlighting the need to understand the complex interactions between the ADC’s structure, internalization, processing, and resistance mechanisms to chemotherapy. Dr Newsom-Davis notes that while some ADCs have shown modest activity, the lack of clear biomarkers and limited immunohistochemistry tools hinder their widespread adoption, and emphasizes the importance of developing bespoke biomarkers to accurately target patients.
