ESMO GI 2025 | BXCL701 plus pembrolizumab in 2L advanced PDAC

Juan Manuel O’Connor, MD, MSc, Instituto Alexander Fleming, Buenos Aires, Argentina, discusses the findings of BXCL701 plus pembrolizumab in second-line (2L) advanced pancreatic ductal adenocarcinoma (PDAC). The compound aims to reprogram the tumor’s immune landscape to enhance anti-PD-1 activity. Preliminary results presented at ESMO GI 2025 showed modest outcomes: 17% partial response rate and 2.3 months median progression-free survival. While these early results are encouraging, researchers acknowledge they’re preliminary and await future data on biomarkers from this clinical trial to better understand the treatment’s potential effectiveness. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2025 Congress in Barcelona, Spain.

I think what’s an interesting phase to trial with this new compound BXCL701 plus immunotherapy. I think there is a very good rationale to increase and also reprogramming the immune landscape of the tumor cell so in order to increase the activity of anti-PD-1. They reported the partial response in 17% of the cases and also 2.3 median PFS, like it’s a little bit modest result.
