ESMO GI 2026 | Non-operative management in dMMR/MSI colorectal cancer: insights from AGEO-NEO-MSI

Annalice Gandini, MD, Université Paris Cité, Paris, France, explores the future of non-operative management in mismatch repair deficient/micro-satellite instability-high (dMMR/MSI) colorectal cancer, highlighting the disparity between patient outcomes following surgery for rectal versus colon cancer. Dr Gandini describes findings from the AGEO-NEO-MSI study, which found that clinical assessment could not predict pathological response, and therefore better staging of patients is required through integration of imaging techniques and translational biomarkers. The study found doublet immune checkpoint inhibitor strategies were associated with high rates of complete response but also higher toxicity, meaning treatment decisions should be made on a case-by-case basis. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2026 Congress in Munich, Germany.
