ESMO GI 2026 | Biological insights and breakthroughs in HER2+ lower GI cancers

Ramon Salazar, MD, PhD, Institut Catala D’Oncologia, Barcelona, Spain, discusses biological insights and emerging breakthroughs in HER2-positive lower gastrointestinal (GI) cancer. Dr Salazar also highlights that RAS and BRAF are important drivers in acquired resistance, hence HER2-targeting agents tend to be more effective in tumors also with mutations in these molecules. New antibody-drug conjugates (ADCs) could help overcome resistance due to bystander effects, but they still require tumors to be HER2-positive for efficacy. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2026 Congress in Munich, Germany.
