ESMO GI 2026 | Continued treatment with HER2-targeted therapies in patients with GEA
Filippo Pietrantonio, MD, Fondazione IRCCS Istituto Nazionale dei Tumori, Milan, Italy, shares insights into how continued treatment with HER2-targeted treatment should be approached in patients with gastroesophageal adenocarcinoma (GEA). There is currently no data on resistance mechanisms to zanidatamab, whilst for trastuzumab, these are more established. Dr Pietrantonio highlights the Destiny-Gastric04 trial (NCT04704934), which evaluated second-line trastuzumab deruxtecan against ramucirumab plus paclitaxel in HER2-positive gastric cancer, and required biopsy confirmation of maintained HER2-positivity following first-line trastuzumab-based treatment. HER2 is not a static therapeutic target, and therefore requires reassessment when feasible across the treatment period. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2026 Congress in Munich, Germany.
